$100 for your evaluation. $70 for each follow-up visit. Those are the self-pay rates at Lake Erie Physical Therapy, our clinic on Virginia Street in Buffalo, for anyone paying without insurance.
If you have been searching for the average cost for physical therapy, you have probably found wide ranges. An average cannot tell you what your own care will cost, because that depends on how you pay, what your plan’s cost-sharing looks like and how many visits your plan of care includes. This page walks through each piece.

We price by the visit, not by the hour. A week with your evaluation and one follow-up visit comes to $170. A week with two follow-up visits comes to $140. How many visits you need, and for how long, is something your physical therapist will go over with you after the evaluation.
Self-pay is also an option if your plan does not cover physical therapy, if you have used up your plan’s visits for the year, or if we are out of network for your plan.
Why there is no single average cost for physical therapy
If you use insurance, three terms decide most of what you pay:
- Copay: a fixed dollar amount you pay for a covered visit.
- Deductible: the amount you pay for covered services before your plan starts to pay.
- Coinsurance: the percentage of the allowed cost you pay after you have met your deductible.
Two other things matter. Out-of-network coinsurance usually costs you more than in-network coinsurance, and we are out of network for most Cigna and Humana plans. Some plans also limit how many therapy visits they cover each year. Copays and deductibles are due at each visit. See the full list of insurance plans we accept.
Keep It Going pricing
After you finish physical therapy, Keep It Going lets you keep exercising with healthcare professionals on staff. Some people also use it for pre-season conditioning.
- $10 per visit, or $70 per month for unlimited visits
- Paid out of pocket; it is usually not covered by insurance
- Requires a prescription for maintenance exercise from the physician who originally referred you, medical clearance to exercise from your family physician, and a signed waiver

How to get an estimate
Using insurance: Call the member services number on your card and ask:
- Is physical therapy covered, and is Lake Erie Physical Therapy in network?
- What is my copay or coinsurance per visit?
- How much of my deductible is left this year?
- How many visits do I have left, and do I need a referral or prior authorization?
Then call us with your card and your answers.
Paying on your own: If you are not using insurance, you have the right to a Good Faith Estimate of expected charges. You can ask for one before you schedule, and when you book at least three business days ahead, the estimate is required. Just ask when you call.
