Keep It Going Extended Care Program

Keep exercising after discharge in a clinic you know: $10 a visit or $70 a month for unlimited visits.

$10 a visit, or $70 a month for unlimited visits. That is the price of Keep It Going, the extended care exercise program at Lake Erie Physical Therapy in Buffalo. If you’ve been looking up senior exercise classes near me because you want to hold on to the strength you built in physical therapy, this program may suit you better than a class. You keep working on your own program, in a clinic you already know, in a space staffed by healthcare professionals.

Why it exists

Finishing physical therapy doesn’t mean you’re done exercising. Keep It Going is for patients who have completed their supervised rehab and want to keep exercising at our Buffalo clinic, maintaining what they gained and building general conditioning in a familiar setting.

Searching for senior exercise classes near me?

Here is how Keep It Going differs from a typical class:

  • It’s not a group class. You work on your own maintenance program, set up at an initial consultation.
  • Healthcare professionals staff it. Think of it as a medically oriented gym for people who have finished rehab.
  • Your physicians are in the loop. Joining requires a maintenance exercise prescription and medical clearance.
  • It’s not only for seniors. Older adults who want regular strength training can use it, and so can athletes getting ready for a season.

Pre-season conditioning

Keep It Going can also be used for pre-season conditioning for a sport or recreational activity, with the goal of improving performance and reducing the risk of injury. If you’re coming off an injury, start with sports physical therapy and move into the program once you’re discharged.

What you need to start

You’ll need three things:

  1. A prescription for maintenance exercise from your original referring physician
  2. Medical clearance to exercise from your family physician
  3. A signed waiver from Lake Erie Physical Therapy

Then call the clinic to schedule an initial consultation, where we go over your health history and exercise goals. If you started therapy without a referral, call us before you finish therapy and we’ll walk you through what’s needed.

Cost

  • $10 per visit, or
  • $70 per month for unlimited visits

Keep It Going is out of pocket and usually not covered by insurance. For physical therapy rates, see our pricing page.

Who it suits

People finishing rehab after a knee replacement or another surgery (see post-surgical rehab), people with back or joint problems who want to stay consistent with their exercises, and athletes heading into a season.

A note on safety

Keep It Going is exercise, not treatment. If a new pain starts, an old problem flares, or your health changes, tell the staff; you may need to check in with your physician or come back for a physical therapy evaluation. Stop exercising and get medical help right away for chest pain or pressure, unusual shortness of breath, or feeling faint.

To set up your initial consultation, call (716) 332-4838, or send a request and mention Keep It Going.

Is Keep It Going covered by insurance?

Usually not. Keep It Going is an out-of-pocket program, $10 per visit or $70 per month for unlimited visits. It is separate from physical therapy visits billed to insurance.

What do I need to join Keep It Going?

Three things: a prescription for maintenance exercise from your original referring physician, medical clearance to exercise from your family physician, and a signed waiver from Lake Erie Physical Therapy. Once you have them, call the clinic to schedule an initial consultation.

Is this a senior exercise class?

No. Keep It Going is not a group class. You work on your own maintenance program in our clinic, in a space staffed by healthcare professionals, whether you finished rehab after surgery or want pre-season conditioning for a sport.

What if I started physical therapy without a referral?

The program asks for a prescription for maintenance exercise from your original referring physician. If you started therapy through New York's direct access rule, call us before you finish therapy and we will walk you through what is needed.
